Payroll and Benefit Information for Current and New Employees

All forms must be submitted before payroll can be initiated

This page contains information for payroll and benefits, as such the forms on it are relevant to staff who are new to Lynnfield Public Schools as well as current employees who wish to make changes to their current pay or benefits. Please read the information below carefully as a few of the forms are specific for an employee's role, such as part-time, non-teaching staff and teaching staff.

All forms must be completed on a computer. We are no longer accepting hand-written forms. Please see sidebar for instructions to complete the forms on a computer. If you do not have access to a computer, please make an appointment with Danielle Leonard who can set up an office computer for your use.

All new hires must complete a finger-printed criminal background check before completing any paperwork. Please ensure that you have done so before proceeding:

  • Please bring your license to Annmarie McCormick at Central Office and she will run your CORI. This process will take 5-10 minutes.

Fingerprint Requirements

  • All employees are required to be fingerprinted at their own expense. This guide explains how to make an appointment to complete this process.
